Claim Department Reporting Services

Any illness, injury, or death of an insured animal should immediately be reported to our office at 630-232-2100.

Notice may be given by the insured, his representative or other person having care, custody and control of the animal. A claim representative is available by telephone 24 hours a day, seven days a week, including holidays.

When making a report, the following information should be provided: policy number; name of insured; name of animal; location of animal; name and phone number of person having care, custody and control; name and number of attending veterinarian.

Please note: Notice must be given for all surgical procedures performed for non-life threatening conditions prior to the surgery being performed. This includes castration.

When a claim is called in to this office after business hours, the caller will receive a recorded message. This message will instruct the caller to contact the Company representative on duty. In the event the caller reaches the representative’s voice mail, a clear and concise message with the caller’s phone number should be left. Our representative will return the call promptly.
